Memorial promotes Vicki Landes to vice president of Community Care Coordination and Services

Vicki Landes, RN, BSN, MBA has been named Vice President of Community Care Coordination and Services at Niagara Falls Memorial Medical Center.

Landes is a graduate of Medaille College, where she was awarded a master’s degree in Business Administration, and SUNY at Brockport, where she earned a Bachelor of Science in Nursing degree.

She has served as director of Memorial’s adult Health Home since 2013 and has more than 17 years of experience in the case management field. Under her leadership, the Health Home has emerged as a dynamic, multi-faceted care management model that has improved health and quality of life for thousands of Niagara County residents.

In her new role, Landes will provide organizational oversight for numerous programs and special projects related to care coordination and community services.  In addition, she will work to integrate Memorial’s care management systems and services to deliver a well-coordinated, patient-centered experience across the continuum of care for patients and their families.
